An assessment is included for Friday that assesses the skills and problem types addressed in that week’s homework.
It has been paper/pencil in the past, but I’ve been transitioning over to Google Forms. The real accountability piece that I grade is the Quiz.
(I used Google forms for other assessments over the past couple of years, and have absolutely loved it! It is directly aligned to the skills practiced on the homework. It took a couple of years to get this routine in place, and the the one thing that helped the most was having a consistent format for my homework. Much like you, I wanted my 4th graders prepared for their End of Year Assessment.
